BNS Survivorship Assistance How to Apply
The Barangay Nutrition Scholars (BNSs) play an important role in monitoring the nutritional status of children and other nutritionally at-risk groups in a community. They are also equipped with enough training in organizing community nutrition activities and disseminating nutrition information in the barangay.

As discussed in the previous articles about the incentives a BNS is entitled to, their beneficiary can also receive a survivorship assistance worth P20,000.00. 

Step 1. Submit the following documents to the National Nutrition Council Regional Office:

1. Endorsement letter from the local chief executive
2. 
Request Letter of the beneficiary 
3. 
Photocopy of government-issued ID of the deceased BNS
        - Must be certified true copy by P/C/MNAO, D/CNPC, or any barangay official
4. Photocopy of valid ID of the beneficiary
5. Death Certificate 
        - Must be certified true copy 
6. BNS master list 
        - Must reflect the following: a) Complete name of BNS, b) Barangay Assignment, c) Birthdate, d) Age, e)
          Gender, f) Deployment Status (Existing/New) g) Date of Employment, h) Inclusive Date of Service, i)
          Name of Beneficiary, and j) Relationship of BNS to Beneficiary.
        - Must be original or certified correct by the P/C/MNAO and signed by the governor or mayor.
        - Accurate number of BNSs must be reflected.

Step 2. NNC Regional Office will process and endorse the request to the NNC Central Office for release of check.

Step 3. Released check from NNC Central Office will be transmitted by the NNC Regional Office to the LGU intended. 

Note: Validity of check is only for three (3) months upon release.

The application must be filed within six (6) months of death of the BNS. However, with the pandemic still ongoing where most logistics and transportation are restricted, submission of requirements has been extended up to nine (9) months to file for claim.
