Cebu City – So you’re done with the weight and height/length-taking portion of the OPT Plus and now you are ready to prepare the report for your local nutrition committee, where do you start?
The first step of course is data entry and encoding of results using the eOPT tool but what next?
There are several forms that one needs to prepare after all the data has been inputted into the form and we are here to help you out with that. With a little introduction to each form.
OPT Plus Form A of the barangay tool serves as the pre-printed list of preschool children in the barangay and will be used as the “master list” for your next weighing/height-taking activity. Barangay health and nutrition workers are encouraged to write down the names of new children in the community and note those who have transferred out or expired.
OPT Plus Form B shows a summary of the community’s nutrition status by showing the number and prevalence of children per nutritional status, age group, and sex.
OPT Plus Form C on the other hand is a list of all affected/at-risk pre-school children under five years of age. It shows the type of malnutrition that affects each child on the list which helps the local nutrition committee identify interventions to provide.
At the barangay level, all three forms must be printed, presented to the local nutrition committee, signed by the BNC chair, the BNS, and the nutrition action officer, then filed for future reference.
